New Patient Application

Thank you for taking a few minutes to fill out this form. This form is for potential new patients who have never seen Dr. Schlick before. If you have previously seen Dr. Schlick at AdventHealth, please use the “Existing Patient” form to continue your care.

Please note: The minimum investment to work together is the cost of a new patient visit, which is $425/75-minute appointment, we do not accept insurance.

Most patients will receive personalized lab recommendations after this visit, which may range from $80 to $200. At the end of your first visit, we will discuss whether it’s more cost-effective to process labs through insurance, HSA funds, or direct payment to the lab. If comprehensive labs are ordered, a follow-up lab review appointment will be essential, costing $225 for 45 minutes. Typically, patients will have 2-3 appointments within the first 6 months and then transition to annual or as-needed check-ins. My goal is to understand your health thoroughly and support you in maintaining and enhancing your health through ongoing monitoring and care.
