If you were a previous patient of Dr. Schlick and wish to continue your care, schedule follow-up , an annual wellness visit, or arrange labs and treatment updates, please fill out this form to help transition your care to my new office.

Leaving AdventHealth was a bittersweet decision, and I’m incredibly grateful that you’ve found your way to my new clinic.

By completing this form, you'll help me expedite the scheduling process and ensure that we have any necessary labs completed before your follow-up appointment. While my practice will remain similar to how it was at AdventHealth, I’m excited to offer more personalized care with longer appointments. Follow-up visits will now be 45 minutes/$225, and new patient visits will be 75 minutes/$425. This additional time will allow me to provide more thorough education and treatment recommendations.
